My nephew is autistic and loves to be in the water so my brother (his dad) taught him to say Okay when he comes up from going under the water (presuming he is okay).

Well they have a giant hill for a back yard so they went sledding. The first run went well and John liked it so they went again. The second time the ending wasn't so good. They crashed - John ended up in a snow bank head first - lost his mittens in it and had snow cramed into his coat. He pops his head out of the snow and says Okay (somewhat hesitantly).

He wanted to go again but Dad made him go inside and dry off before Mom had his hide.

'Sounds like the poor little guy tho't he should say, "Okay," to let them know he was alive ... but wasn't sure, not feeling too "Okay" in that predicament! Cute! ;)
